USING AN AGENT TO SUBMIT A REQUEST

You may use an agent to submit your request, if allowed by law. If you use an agent, we may require you to send us a power of attorney. If you do not send us a power of attorney, you may be required to provide commercially reasonable identification of your identity and evidence of the agent’s authority to act on your behalf. APPEALS
